List of requests:

• Prison Details page

  • Probation officer name – change to include contact details or add boxes for contact.
  • Prison In – Change to ‘Length of Sentence’.

• View Participants page

  • Add in headings on table ‘full name’ and ‘surname ‘age’ and ‘borough’.
  • Remove CSCS support from this page and add to the list of interventions.
  • Change disability and medical field to Do you consider yourself disabled. If yes then please provide a free text field for disability to be entered.

• Add intervention page

  • Make "add intervention" choice just for admin.
  • Add notes as a column title – Interventions- start- end – type – action – then add notes to this line.

• View Managers Page

  • Limit Manager details displayed to just Name, Email, phone number.Please remove –
    • gender,
    • marital status,
    • Dob,
    • Full address
    • post code.

• Worker Page

  • On current worker please make same information as revised managers’ page.
