After PR #144/#222 deploy, exercise VOICE end-to-end: unenrolled users must NOT be
routed into voice; enrolled users pass; bio-outage fails OPEN (kill-switch
APP_AUTH_ENROLLMENT_PROBE_ENABLED). Live GUI + persistence per verify-in-real-product rule.
