On page 2, questions #3 of the Annual Care Plan/Report, it asks “Is the Incapacitated Person taking and/or receiving antipsychotic medication(s)?” I have created a pop up that explains by answering yes to this question, the guardian will need to notify the court because it adds a “Rogers Guardianship” requirement.

Additional language should be added to this pop up to (1) briefly explain the rogers guardianship process, (2) provide a hyperlink to a helpful website that includes a list of common antipsychotic medications, i.e., abilify, and (3) tells the guardian that if they are unsure whether the incapacitated person is on antipsychotics, that that information may be included on the “Clinical Team Report” or “Clinician’s Affidavit” that they filed with the court for the guardianship already.
