[FRONTEND] Add responsive layout to Verification Page

Description

The Verification Page input and result badge are not optimized for mobile. On small screens the input field and badge overlap or overflow. The layout needs a mobile-first redesign.

Acceptance Criteria

• Input field is full-width on mobile
• Verification badge stacks below the input on screens < 640px
• Result metadata (vaccine name, date, issuer) wraps cleanly
• Page is usable with one hand on a 375px screen
• No layout shift when the result loads
