New Claude chat resets the permission setting

Testing #312653

• new VS Code window on an empty folder and on a new profile
• open chat, select Claude Code and Ask before edits
• do some prompting
• open chat (with the + in the tool bar).
• New chat opens with Claude (good) but has switched the permissions back to `Edit automatically'
• I really don't want Claude to auto permit. So IMO the best is to use the last used permission settings, or have a setting like chat.permissions.default

yeah i think it's the default if the session doesn't have a value already, but we can have a "last used"
