Set up a team schedule and guidelines for production failures

Ideally we would have a system in place where someone is nominally "on call" for cataloging production failures and taking action if they're critical. There should be a standard process to follow, and possibly (depending on workload) typical duties relating to maintenance for the on call person.

I'll take a stab at a proposal, and we can iterate democratically from there
