WHY this matters

Key bug fixes that improve reliability:

Windows VS Code extension now works - Fixed activation failure due to bundled SDK createRequire polyfill bug
Authentication more reliable - Fixed Mantle endpoint failing with missing x-api-key header

Plus routine maintenance updates (254 files).
