- Removed outdated CHANGELOG.md file.
- Added new changelog entries for versions 0.4.2, 0.4.3, and 0.4.4, detailing significant changes and bug fixes.
- Fixed a bug in ToolReference to prevent cross-agent trigger filtering issues.
