Add a per-session, append-only transcript table for live Claude session output. `seq` is a monotonic per-session sequence supplied by the producer; the @@unique([sessionId, seq]) makes ingest idempotent (a retried POST with the same seq is a no-op) and the @@index([sessionId, seq]) backs cursor reads (`where seq > after order by seq`). Migration is purely additive (single CREATE TABLE + indexes + FK, no changes to any existing table) so it cannot conflict with in-flight schema.prisma migrations.
