Summary

wire startup memory phase-1 extraction into Session startup flow
persist phase-1 outputs and schedule per-cwd consolidation subagents
add phase progress logs and consolidation lock release-on-agent-completion flow
set consolidation subagent cwd to memory root to align workspace-write permissions

Testing

just fmt
cargo test -p codex-core --lib codex::tests::resumed_history_seeds_initial_context_on_first_turn_only
