Fix: improve README clarity, grammar, and formatting

This PR improves the README by fixing grammar issues, improving sentence clarity, and correcting formatting inconsistencies.

Changes include:

• Improved wording for better readability
• Fixed grammatical errors (e.g., "its" → "it's", "on to" → "onto")
• Corrected numbering in table of contents
