[codex] Add report feedback links

Summary

• add feedback links to Markdown reports so Action step summaries point users to false-positive, missing-signal, team-pilot, and validation tracker paths
• cover clean Markdown reports with a CLI regression test

Scope

Only touch Markdown report rendering and CLI output tests so Action step summaries link to false-positive, missing-signal, team-pilot, and validation tracker feedback paths.

Validation

• red: node --test test/cli-output.test.mjs failed because Markdown reports lacked the feedback footer
• node --test test/cli-output.test.mjs
• npm.cmd test
• npm.cmd run build
• git diff --check
• local TaskBound review returned NONE for the PR scope and rendered the new footer
